How we collect, use and protect your personal information.
Our commitment
Max Physiotherapy respects your privacy. This policy explains how we collect, use, share and protect personal information through this website, our online booking and our clinic in Calgary, Alberta.
We handle personal information under Alberta’s Personal Information Protection Act (PIPA). Health information collected by our regulated health professionals is also protected by Alberta’s Health Information Act where it applies, and by the standards of each practitioner’s professional college.
What we collect through this website
• Enquiries: your name and phone number, and if you choose to give them, your email address, the service you are asking about and your message. • Online bookings: your name, email address and phone number, the service, practitioner and time you choose, and any scheduling note you add. • Visits to the website: the pages viewed, the buttons and phone numbers pressed, and the website that sent you here. We do not use cookies to track you. Your internet address and browser details are turned into a coded value that changes every day and is used only to count visits.
Please do not send medical history, health card details or clinical documents through website enquiries or booking notes.
What we collect at the clinic
When you come for care, we collect the information we need to assess and treat you. If you are coming about a new condition or injury, this includes a Patient History Form completed at the clinic. We also keep treatment records and the details needed for billing, including insurance, WCB or Alberta Health Services information where it applies.
How we use your information
• To reply to your enquiry • To book, confirm, remind you of, change or cancel your appointments • To assess, treat and care for you • To bill you, your insurer, WCB or Alberta Health Services, with your consent • To meet our legal and professional obligations
We use your information only for these purposes, or for another purpose with your consent or as the law allows.
Consent
By sending an enquiry, booking an appointment or attending the clinic, you consent to our collecting and using your information for the purposes above. You may withdraw your consent at any time, subject to legal or contractual limits, by contacting us. We will explain what withdrawing consent means for your care or appointments.
Who we share it with
Your information is available only to our practitioners and staff who need it to care for you or run your appointments.
We share it outside the clinic only with your consent or as the law requires or allows: for example, with your insurer or WCB for direct billing, or with another health provider you ask us to work with.
We use service providers for this website, online booking and email. Some of them store or process information outside Canada, including in the United States, where it may be subject to the laws of that country. You can contact us to ask about how these providers handle your information.
We do not sell or rent your personal information.
Maps and videos
Some pages show a map from Google or a video from Wistia. When those load, Google or Wistia may collect information such as your internet address and set their own cookies, under their own privacy policies.
How we protect it
We protect personal information with reasonable physical, electronic and administrative safeguards, and keep it only as long as needed for the purposes above and as the law and our professional colleges require. If a privacy breach creates a real risk of significant harm to you, we will notify you and the Office of the Information and Privacy Commissioner of Alberta, as the law requires.
Your right to access and correction
You may ask to see the personal information we hold about you and ask us to correct it. Please make your request in writing to our Privacy Officer, Gurvinder Singh. We will respond within the time the law requires, and if a fee applies, we will tell you before we proceed.
